Utility Forms
The following are forms that you can use to set-up and/or update your utility billing account information:

Change request
For residents who need to update the personal information on their account (such as changes in names, mailing address, phone numbers, etc). This form will need to be printed, filled out, and signed by the appropriate parties. The completed form and supporting documentation can be returned in person to City Hall, faxed to 780-871-8345 or emailed to .
   
Connection
For residents who do not currently have a water account with the City of Lloydminster.
   
Disconnection 
For residents who no longer require water services in Lloydminster. 
   
Transfer 
For residents who are moving from one house to another within the City of Lloydminster.
   
Pre-authorized Payment For residents wishing to set up pre-authorized utility payments in the City of Lloydminster.
   
Water meter read
Submit a water meter read.

Once you have completed and submitted the forms, they will be directed to the Water desk for processing. A representative from the Finance Department will follow up with you within one business day regarding your submission.

To ensure that water meters are read in conjunction with moving dates, please submit forms at least three business days prior to moving. The City reads water meters on weekdays so if you are moving on a weekend, your meter will be read the Friday before or the Monday after you have moved.
