
Lloydminster, AB/SK – The Government of Saskatchewan announced that proof of vaccination or negative test results would be required to enter several establishments, businesses and event venues come Friday, October 1, 2021. The province also announced an interim mandatory masking order effective Friday, September 17, 2021.
How do I get my COVID-19 vaccination records?
Alberta Residents
All immunizations administered in Alberta will be automatically updated to your MyHealth Records account within two to three weeks of the doses being given.
Download a copy of your vaccine card at Alberta.ca/covidrecords.
Vaccinated Outside of Alberta?
Any Albertan who has received a COVID-19 immunization outside of the province, or outside of the country, can upload their immunizations for review and verification, to then be added to MyHealth Records.
Out-of-province and out-of-country immunization records can be submitted on behalf of yourself or your child/youth under 18 years.
To upload your records, visit AHS.ca/vaccineregistry.
Saskatchewan Residents
Saskatchewan residents who would like a record of their COVID-19 Vaccination Record now have access to a new one-page printable form from their MySaskHealthRecord account at eHealth Saskatchewan. A scannable QR code is also now available.

Vaccinated Outside of Saskatchewan?
If you have been vaccinated outside Saskatchewan and want your immunization information captured to your Saskatchewan health record, visit eHealth Saskatchewan’s website or call at 1‑844‑767‑8259 (and press 3) to learn more.
You will be required to fill out a form and provide a copy of your immunization record/documentation. This applies to first/all doses administered outside Saskatchewan. Your Saskatchewan health record should be updated in three to five business days of providing the required information and documentation.
